Use this form to notify a homeless applicant of the outcome of the initial section 62 assessment, where you have decided the applicant is homeless, but you consider that the conditions allowing the referral of the applicant's case to another local authority are met.

The form prompts you to confirm all matters that councils are required to notify applicants of, under section 63 of the Housing (Wales) Act 2014.
