This letter enables you to forward details of a homeless applicant's case to social services, where the applicant has consented to a referral under section 96 of the Housing (Wales) Act 2014.

A referral under s.96 does not have to done by letter. However, homeless officers may find it useful to do so.

The letter includes a brief explanation for social workers of why the referral is being made (i.e. that you're duty bound to do so under section 96(2)(b)).
