Use this form to notify a homeless applicant of the outcome of the initial section 62 assessment, where you have decided the applicant is homeless, and you have reason to believe that the applicant may have a priority need.

Use this form when you're accepting the section 68 interim accommodation duty.

The form prompts you to confirm all matters that councils are required to notify applicants of, under section 63 of the Housing (Wales) Act 2014.
