A letter that a local housing authority in Wales can use to:

• Confirm the date and time of a homelessness assessment interview.
• Ask the applicant to complete an application form, and
• Ask the applicant to provide supporting documents, e.g. to evidence their identity, nationality, proof of residence etc.

The letter is designed to be used in conjunction with:

• An application form (which can be enclosed), and
• A letter confirming an offer of interim s.68 temporary accommodation (if the s.68 duty is owed) (see 'Related documents').
