A form asking for detailed information from a GP or doctor about a homeless applicant, as part of your inquiries under section 184 of the Housing Act 1996.

The form will elicit the medical information you need to decide:

• whether the applicant is vulnerable, and
• whether there are any specific requirements regarding accommodation, in order for it to be suitable for the applicant.
