Use this letter template to:

• notify another local authority that you consider the local connection referral conditions are met in respect of an applicant who has applied to your council as homeless, and
• request that they confirm whether they agree that the referral conditions are met.

Note: this template should be used in conjunction with the standard referral request form contained in the Code of Guidance.
