Requesting medical report re vulnerability
A letter asking your client's doctor or health professional for information, so as to support a homeless application to a Welsh council.
The letter includes questions that should elicit information relevant to:
- whether the applicant is vulnerable, and
- whether any particular form of accommodation is required.
The letter also asks for a copy of your client's medical records.
