A form for notifying a homeless applicant of how you've assessed their case under section 189A of the Housing Act 1996 when:

• conducting the initial assessment (as opposed to reviewing the assessment - separate letters and forms are available for this), and
• deciding the applicant is threatened with homelessness and owed the s.195 prevention duty.

The form includes spaces for confirming:

• what kind of housing the applicant needs
• whether there are any support needs, and
• your reasons for deciding the applicant is not homeless.

This form is designed to be accompanied by a covering letter and housing plan, which are available separately.
